Better Health and Wellness Through Partnership
The Health Office website is a central resource for students, families, and staff to access key information, forms, and policies related to student health. Here, you will find everything from medication guidelines and illness protocols to wellness support and school health requirements. Our goal is to provide clear, up-to-date guidance to help the school community stay informed and prepared.
We believe student health is a shared responsibility. A strong partnership between families and the Health Office plays a vital role in each child’s success. Open and ongoing communication allows us to deliver responsive, individualized care and encourages students to take an active role in their well-being. By working together, we can help every student feel confident, supported, and ready to thrive throughout the year.

It's that time of year for updating Medical Records! We sincerely appreciate the time and effort families invest in completing and submitting these forms. Your partnership helps us provide the best possible care for your child and ensures they are ready to fully participate in school life.
All forms may be found on the right side bar of this page under "Student Health Forms," which consists of drop down menus detailing each of the forms and how to submit them.
The following medical forms are required and must be submitted and kept up to date with the Health Office for the new school year:

Emergency Card: This card provides essential contact information, medical history, medication permission, and emergency instructions. It must be completed annually and kept current throughout the school year.

Physical Examination: A valid physical exam must be on file, completed within the past two years. This is required for school enrollment and participation in physical education and athletics. Please ensure that the form is signed by a licensed healthcare provider.

RETURNING STUDENTS: If you submitted an updated physical last school year (dated 2024), you DO NOT need to submit this form this year and can send the Health Office an updated physical once the student receives their next annual physical.

PRESCHOOL/PRE-K FAMILIES: State licensure requires that all Preschool and Pre-Kindergarten students have documented lead testing as part of their health records. This is required before attending school.

DIAA Forms (Delaware Interscholastic Athletic Association): Only required for all students participating in interscholastic sports (Grades 6-8). Please make sure all fields are completed including physician, parent, and student signatures.
Submission Deadline: August 8, 2025
In order to allow adequate time to review, organize, and verify each submission, we kindly ask that families complete all forms prior to the deadline. Your cooperation ensures we are fully prepared to support student health and safety at the start of the school year.

The Health Office is dedicated to supporting the health and well-being of all students throughout the school day. Below is important information about our hours and procedures:
Hours of Operation
The Health Office is available for all students and faculty/staff daily from 8:30am-3:30pm, when school is in session.
Student Visits
Throughout the school day, the Health Office provides care for students experiencing illness, injury, or other health-related concerns. Each visit is carefully evaluated to determine the appropriate level of care. Most minor issues can be managed at school, but families will be notified if a student experiences a more significant illness or injury, needs to be sent home, or requires additional follow-up. Every visit is documented to ensure consistent and coordinated care. Our goal is to support student well-being while minimizing time away from learning.
Communication
Parents will be notified via phone call, email, or in person regarding all Health Office visits that go beyond the scope of basic first aid. This would include head bumps, medications being given, musculoskeletal injury, and any illness that results in multiple Health Office visits.
When a student visits the Health Office with a health concern that requires immediate pickup, parents or guardians will be contacted directly. If a student is sent home due to illness or injury, we ask that they be picked up within a reasonable amount of time (typically within 30–60 minutes) to prioritize their comfort and to limit the spread of illness to others. Timely communication and cooperation help us provide the best care for all students and maintain a safe, healthy school environment.
